Abstract
This paper explores contemporary practice in the use of restraint with particular reference to the question of safety in the light of recent European Council Guidance endorsing the principles of least restrictive environment least intrusive intervention. Background: Violence is a source of concern in many European mental health services but approaches to its immediate management continue to display remarkable diversity reflecting it appears the adoption of differenting perspectives on what constitutes 'least intrusiveness'. Method: The paper examines the basis for practice in terms of the range of procedures available, reviews the literature on restraint safety and discusses the implications for policy, practice and future research. Results: Current practice in Europe is the product of different beliefs in different states (which reflects individual local values) and is not evidence based. Conclusions: Further studes are needed to determine the relative risks related to all the measures in question so that the practice is guided by evidence rather than individual opinions.
